This easy-to-use meeting minutes template helps you document QAPI discussions, decisions, and action items from each meeting. Clear records ensure nothing gets missed and that follow-up actions are tracked effectively.
How to Use This Tool:
During QAPI meetings, use the QAPI Meeting Minutes template to record discussions, decisions, and next steps.
Performance Indicators: This section is intended to be used to discuss performance indicators that you are routinely monitoring, using the QAPI Data Tracking Spreadsheet, that did not meet established benchmark. For each area not meeting benchmark, discuss current performance level, the established benchmark, and complete the Project Prioritization Worksheet (located on Tab 3 of the QAPI Data Tracking Spreadsheet) for process improvement tool to determine the prioritization score, which will be used to help you identify priority areas to establish performance improvement plans for.
I often see facilities talk about all data they are monitoring during the QAPI meeting, which often leads to ineffectively identifying priority areas for improvement. By only discussing areas that you are monitoring that did not meet established benchmarks, you are able to focus on what is important for process improvement.
New Areas Requiring Monitoring: As a team, discuss if there are new areas that you are not currently monitoring in your routine data collection that needs to be added based on feedback received since last meeting.
Areas Selected by QAPI Team for PIP Development Based on Prioritization Scores: Determine areas for Performance Improvement Plan (PIP) development based on the prioritization scores from page 1. As a team, establish goal for the PIP, develop the PIP Charter, select a PIP team leader to lead the PIP project, determine suitable team members for the PIP team, and provide a copy of the PIP Charter to the PIP leader to develop and carry out the PIP with the PIP team.
Review of In-Progress PIPs: As a team, review all current PIPs that are in progress. Review date PIP started, current phase of the PIP project, discuss the purpose of the PIP, new actions initiated since last meeting, data tracking, and current performance in the area.
Recommendations from Committee: Open the floor to committee members for any further recommendations or areas to discuss.
Review of QAPI Plan: Take time to review your written QAPI plan with the team during each meeting to determine if any updates are needed.
Sign-In Sheet: Attach copy of sign in sheet with signatures of all committee members in attendance.
Review minutes from previous meeting at the start of the next meeting to track progress.
